Pre-Application Form

The cycle for pre-application submissions is from Oct. 1–April 30 annually.

Submitting the pre-application form is a separate process from applying for admission to the program. Candidates are required to complete the pre-application form first.

Additional Requirements

The Summary Sheet and Legal Disclosure Forms must be completed when submitting a pre-application. Candidates are asked to complete the forms and send to the MSMS Review Committee via email. Pre-applications will be on hold and considered incomplete until these forms are received.
