Indiana University School of Medicine requires all incoming medical students to meet specific immunization and health requirements for matriculation.
All students must provide documentation for the following:
- Physical examination
- Hepatitis B vaccine
- MMR vaccine
- Meningitis vaccine
- Tdap vaccine and booster, if needed
- Varicella vaccine
- IGRA blood test for TB screening (IU School of Medicine and IU Campus Health no longer accept the 2-step TST test for TB testing)
- Influenza vaccine (required annually in the fall; additional information will be provided)
- COVID vaccine (one current vaccine dated Sept. 1, 2024 or later is required)

- IU Campus Health offers appointments for matriculation immunization requirements and physical examinations. To schedule an appointment, contact IU Campus Health at healthfi@iu.edu.
- IU Campus Health can also provide instructions for submitting documentation received from another healthcare provider.

Medical and Non-Medical Exemption Request
Indiana University School of Medicine recognizes there may be medical or personal, non-medical reasons that lead an individual to request an exemption from the immunization requirement process.
Request a medical exemption
- Email IU School of Medicine Compliance at compreq@iu.edu requesting the Medical Waiver Request for Immunization form.
- Complete the form, include appropriate medical documentation from your provider(s), and email to healthfi@iu.edu cc: compreq@iu.edu.
- Campus Health will review and determine if a medical exemption is granted. They will communicate that decision to the student and IU School of Medicine Compliance.
- A note will be added to the student’s MedHub record stating a medical exemption has been granted for the appropriate vaccination(s).
Request a personal, non-medical exemption
- Students requesting a waiver from one or more vaccination requirements due to personal, non-medical reasons should email compreq@iu.edu and identify which vaccination(s) they are requesting to be waived.
- The student will receive a waiver request form to review and complete. Once completed, the form should be submitted to compreq@iu.edu for review.
- Students are welcome to request a brief Zoom meeting with IU School of Medicine Compliance and/or school leadership to review the waiver request process, discuss the form and ask any questions they may have regarding the request.
- Following review of the waiver request, a note will be added to the student’s MedHub Immunization record documenting the status of the waiver request.
- Students may be required to renew their waiver request annually and provide updated documentation or information, as determined by IU School of Medicine and/or affiliated clinical training sites.
While the IU School of Medicine recognizes that students may choose not to receive a particular vaccine, students are required to meet the requirements, including immunization requirements, of affiliated clinical facilities where they complete required clinical training. Failure to comply with the clinical affiliate's rules may jeopardize the student’s ability to complete graduation requirements. IU School of Medicine will make reasonable efforts to minimize disruption, but cannot guarantee students will not be affected or that they will earn a degree.
