Care Coordination Training Curriculum

The Indiana Complex Care Coordination Collaborative (IC4) offers three current approaches to curricula:

Care Coordination Coaching Curriculum

The Care Coordination Coaching Curriculum accelerates the acquisition of competency for nurses or social workers in their roles as care coordinators. It focuses on the development of real world, point-of care skills to empower the delivery of comprehensive, family-centered and targeted supports for patients and families. The curriculum includes orientation, coaching and high intensity didactics and then continues through participation in a collaborative community of practice.

Primary Care Clinician Curriculum

The Primary Care Clinician Curriculum builds advanced medical home skills in chronic care management and accommodative, inclusive healthcare. It focuses on light touch methods for the busy clinician and includes learning collaborative sessions, maintenance of certification quality improvement projects and easy access to fingertip tools.

Aligned Care Support Network

The Aligned Care Support Network Curriculum connects cross-sector professionals working with families to improve both access to services and quality of care for children and families. It includes asynchronous virtual didactics participation which yields digital badging recognition on completion of successive levels of training and participation in collaborative communities of practice.
