The Indiana University School of Medicine IC4 Technical Assistance Center is a transdisciplinary team of skilled nurse care coordinators, social workers, clinicians and patient and family representatives. Together they have the advanced expertise to train and support new and curious care coordinators to improve the delivery of patient-and-family-centered care which assesses and addresses the comprehensive needs of patients, families and/or caregivers. Their targeted training curricula focus on serving children with medical complexity (CMC) and/or adults with intellectual or developmental disabilities (AIDD). The center is housed within the Department of Pediatrics Division of General and Community Pediatrics.
The infrastructure of the technical assistance center includes:

Formal care coordinator curricula and coaching to promote job success and satisfaction and the professional advancement of care coordination as a field of work.

Orientation, a high intensity curriculum and a longitudinal learning collaborative accelerate the acquisition of care coordinator skills to achieve competency in key milestones and provide high quality care.

Individualized coaching in case discussions exemplifies care coordination processes and is reinforced through chart auditing, quality indicator feedback and competency milestone assessment.

Accurate up-to-date information to facilitate navigation of health systems and government and community services is synthesized and distributed to care coordinators and families.

Patient and family engagement models to apply patient/family-centered principles at all levels of the program.

Patient and family representatives are contributing members of leadership teams and advisory groups.

Communication skill training emphasizes an individualized, tailored approach to Identify needs and strengths and to adapt to preferences and priorities for actions on patient and family goals.

Partnerships with family support agencies provides individual family consultations, information sharing and group activities through Indiana Family to Family and Be Strong Families Parent Café’s.

Semi-annual family satisfaction surveys are collected and synthesized to deliver anonymized feedback to primary care practices.

Primary care team support and education to build advanced medical home skills to address chronic care management for children with medical complexity and healthcare inclusion with needed accommodations for adults with intellectual/developmental disabilities.

Frontline clinical tools for care and care management of key populations are shared.
